EFA805 SELECT TEAM — PARENT & PLAYER AGREEMENT
October 1, 2026 – September 1, 2027
Welcome to Elite Futsal Academy 805.
Our select teams provide a committed training and competition environment where players can develop their skills, confidence, and understanding of futsal. By accepting a place, players and families agree to the following expectations.
SEASON COMMITMENT
We ask families to join with the intention of completing the full season, from October 1, 2026, through September 1, 2027. However, membership may be canceled at any time.
Players are expected to attend practices, games, and tournaments whenever their schedules permit. We recognize that players may have club soccer and other sporting commitments. Please communicate known conflicts early so coaches can plan sessions and rosters.
1. MEMBERSHIP & PAYMENTS
Training membership costs $135 per month for four one-hour sessions. Players must enroll in monthly automatic payments or opt to pay quarterly which is pro-rated fee to a slightly lower rate (3 months). Families agree to the fees and payment schedule provided at registration.
Uniforms, league games, tournaments, governing-body registration, travel, and accommodation involve separate or additional costs.
2. FUNDRAISING
EFA805 is a nonprofit organization. Fundraising helps cover team expenses, equipment, facility costs, and academy operating costs.
Families are expected to participate in designated team fundraising activities throughout the season. Participation requirements and deadlines will be communicated in advance. If your family is unable to participate, please contact EFA805 privately to discuss your circumstances.
3. CANCELLATIONS & MEMBERSHIP PAUSES
Membership may be canceled at any time. Cancellation stops automatic payments beginning with the next billing cycle, provided it is completed before that cycle begins. No refund will be issued for unused prepaid training. To cancel or pause membership, log into your account at www.efa805.com.
Players may pause membership once per season for up to 30 days. Pause requests require at least one week’s notice. During an approved membership pause, players remain eligible to participate in games. However, players with active memberships will receive priority for game opportunities. Players whose memberships are paused may receive less playing time.
If your child is injured, please notify EFA805 promptly so we can discuss an additional membership pause.
4. MISSED SESSIONS, MAKE-UPS & GAME CREDIT
One missed practice per month may be made up on another training day, subject to availability, or exchanged for one game credit.
• Make-up sessions must be completed within four weeks of the missed practice.
• One practice credit covers one game credit and may be used toward any friendly game participation hosted by EFA805 or 805 Futsal League game.
• Absence notice must be provided through TeamSnap or directly to EFA805 by 1:00 PM on the day of practice.
• If neither TeamSnap nor direct notification is received by the deadline, no make-up or game credit will be offered.
Please arrange make-up sessions and game credits with EFA805 before using them.
5. EFA805 TRAINING CANCELLATIONS
If EFA805 cancels training because of weather or facility issues, a make-up session will be offered on a different day. This may require date and time change.
6. TEAMSNAP & COMMUNICATION
TeamSnap is our primary platform for schedules, availability, rosters, and team updates. Parents are responsible for checking it regularly.
Practice availability should be marked 72 hours minimum in advance, with any subsequent absence reported by the 1:00 PM deadline on practice day. Tournament availability must be submitted by the deadline provided.
If availability depends on another commitment, select “Maybe” and include the details in the notes. Notify the coach directly if availability changes after a roster has been announced.
Players whose availability is unmarked may be left off the roster. Coaches cannot follow up individually with every family each week.
7. TOURNAMENT FEES & COMMITMENTS
Tournament registration fees are divided equally among the players attending.
Coaching expenses for all coaching staff attending the tournament are also divided equally among participating players. These include coaches’ game fees, hotel accommodation, mileage, and a food allowance.
Tournament fees do not include each family’s personal expenses, such as travel, hotels, parking, or meals. Estimated player fees will be communicated before families confirm participation.
Players are expected to attend tournaments whenever their schedules permit. No tournament fee is required for a player who opts not to attend. Once participation is confirmed and the tournament fee has been paid, payment is nonrefundable, with no exceptions.
Coaches rely on confirmed participation when finalizing rosters. Please report any subsequent conflict immediately.
Players wishing to guest play with another futsal club must obtain approval from EFA805 before committing. Requests will be reviewed individually, considering the player’s development and existing team commitments. If events conflict, players must prioritize their commitments to EFA805.
8. FRIENDLY GAMES & LEAGUE PLAY
Friendly games and 805 Futsal League league play generally cost $15–$20 per player, per game. This covers applicable entry fees, facility costs, referee fees, and coaching fees.
Fees may vary depending on the league, event, or tournament. Any changes will be communicated before families confirm participation.
9.TEAM SELECTION & PLAYING TIME
A select-team place does not guarantee selection for every event. Coaches consider attendance, effort, attitude, player development, positional requirements, and the needs of the team. We aim to provide meaningful playing opportunities while considering player development and the needs of each game. Equal playing time is not guaranteed.
10. UNIFORMS & EQUIPMENT
All players must purchase:
• Full uniform bundle: $160
• Training jersey: $35
• Full goalkeeper uniform: $80
Players must wear the designated EFA805 training uniform (practice jersey and gas shorts) for practices and game uniform for competitions.
Players must bring suitable futsal/indoor shoes, shin guards, and water.
Parents must inform coaches of injuries, relevant medical conditions, or restrictions that could affect participation.
11. PLAYER CONDUCT
Players are expected to:
• Respect coaches, teammates, opponents, referees, and facility staff.
• Listen, work hard, and contribute positively to the team.
• Demonstrate good sportsmanship.
• Use appropriate language and care for equipment and facilities.
• Represent EFA805 positively at events and online.
Bullying, discriminatory comments, threatening behavior, and physical aggression are unacceptable. Conduct concerns may result in a family discussion, removal from a session or game, suspension, or removal from the program, depending on the circumstances.
12. PARENT & SIDELINE CONDUCT
Parents agree to encourage all players and maintain a supportive environment.
Please allow coaches to coach and players to make decisions during games. Avoid giving tactical instructions from the sidelines or confronting referees, opponents, or other families.
Questions about development, selection, or playing time should be discussed privately with the coach at an appropriate time, away from players.
13. SCHEDULE & FACILITY CHANGES
Training times and locations may change due to facility availability, weather, seasonal daylight, or competition schedules. Updates will be communicated through TeamSnap. EFA805 cannot guarantee that every practice and game will accommodate each player’s individual schedule.
Players should arrive ready to train at the scheduled start time. Game arrival times will be listed in TeamSnap.
14. EMERGENCY CONTACTS & MEDICAL INFORMATION
Families must provide current emergency contact details and relevant medical information at registration, including allergies, medications, injuries, medical conditions, or participation restrictions.
Parents/guardians are responsible for keeping this information current and notifying EFA805 promptly of any changes that may affect their child’s participation.
ACKNOWLEDGMENT
By signing and submitting the registration form, the parent/guardian confirms that they have read and understood this agreement, including the season expectations, payment options, cancellation and pause procedures, make-up and game credit policy, tournament payment terms, uniform requirements, and conduct expectations.
We agree to communicate promptly and help maintain a positive environment for every player.
