PARTICIPATION REQUIREMENTS

Elite Futsal Academy 805

Participation in any training session at Elite Futsal Academy 805 requires completion of the appropriate documentation prior to participation.

All players participating in select teams, academy training sessions, private training, team training, or Lil Elites must complete:

• Waiver & Release of Liability

• Medical Authorization

• Required registration information

The waiver and release form must be completed in full before a player is permitted to participate in training.

In addition to the Waiver & Release of Liability, Select Team players and parents are required to review and agree to:

• Parent Code of Conduct

• Competition Travel Policy

• Select Team Game Time Policy

• Select Team Commitment Agreement

Completion of these policies is mandatory for roster placement and competition eligibility.

Participation in training or competition constitutes agreement to all applicable policies and requirements.
